How to Sign Up

Now that you know how useful a Patient Portal account can be, let's discuss how you can make one for your family. Signing up for our patient portal is free, and it's easy! Below is an overview of the Patient Portal registration process.

Step 1

Patients will receive a registration email from Pristine.

Step 2

Fill out the information needed.

Step 3

After filling out, the form you will be redirected to the Patient Portal homepage.
  • Registration form.
  • HIPAA forms.
  • Authorization for release of info form.
  • And So on..

Pristine will be open tomorrow, Wed Jan 22nd, for the clinics at the following times, shown below:

Wednesday, Jan 22nd
Main = OPEN @ 11am
Spring = OPEN @ 11am
Cypress = OPEN @ 11am

Important Message

We here at Pristine take great pride in patient care. We are updating our current electronic medical record to a new and improved one! This new EMR will allow for a more efficient streamlined process for your care delivery.

As with any new process, there may be some challenges along the way. Please be patient with us as we work through this transition.

We strongly encourage you to register for the new patient portal and complete all your pre-visit check in tasks on your phone at least 30 minutes prior to your visit.

If you had a patient portal prior to December 9th 2024, you will need to re-register for a new portal.
